Where Do OS Updates Come From When You Have An Independent Carrier?

I have a 3 month old Note 2 that came with 4.1.1 and last March, I got the update to 4.1.2.
My question is... where do the updates come from with an independent carrier.
My Carrier uses AT&T, but with the 4.1.2 update, the notification only said that there was an update, and not from any particular source, and I'm just wondering if it came from AT&T.
I registered my phone with Samsung, and of course I'm all up to date with Google... so I just wonder where the updates come from.
A few days ago I got 17 (Seventeen!!!) updates on my phone which makes me think that an OS update is right around the corner for me.

My Note 2 is an international model, unlocked and no SIM, and works fine except for the call volume problem that I mentioned in my first post.

I just discovered this site, like three days ago and really great site.

Being unlocked/carrier free, I believe the updates would come directly from Samsung?

I have a GSM Galaxy Nexus and took it out of the box and put an AT&T Sim card in. Phone worked fine and after a week I got an update which had to come from Google, as AT&T never offered the Galaxy Nexus. If you got an update, it must have come from Google, too.

@ drdebian... I didn't mean to say that my Note wouldn't accept a sim, I just meant to say that the phone came without a SIM... as like in a phone from a major carrier.
Sure, I have a SIM in there, but one from Consumer Cellular.

By the way, I'm getting the phone replaced because of phone call volume issues.
I have a Motorola Bravo and I could never listen to that phone with the volume cranked up to maximum, but this phone sux as far as phone call volume is concerned... even on speaker phone setting.
I can't hear anything while pushing my cart thru WalMart. LOL. I have to keep saying something like... "Run that past me once again". LOL.
For the "Flagship Of The Fleet", you'd think that the call volume would be a bit more hearable.
Don't worry.... I'm gonna post a comment on International models regarding volume.
